John Webster's Duchess of Malfi has one, and he's....memorable.

tiny, dollhouse-sized book contains the short descriptive speech of a
horrified bystander from Act V, scene ii.The book is illustrated with
the skulls of men and wolves. And the detached leg business is not the
most gruesome thing the the play, or even in the act. Strange,
beautiful, haunting, and grotesque.
The book is bound black paper, with decorative endpapers and a paper title pasted on the front.
